What is MyChart?

MyChart is a secure, online patient portal that connects patients to parts of their CHEO electronic health record, anywhere, at any time. CHEO wants to give children, youth and families the most up-to-date health information available — even after they leave the hospital. This is why we offer MyChart.

Through MyChart, you will be able to see information related to most of your visits. MyChart is more than just an electronic version of a patient's chart. It provides:

  • easy access to patient information
  • information that patients and families need to make decisions
  • help empowering parents to be true partners in their child's care
  • information to help families keep on top of upcoming appointments
  • support to youth moving to adult care

How to sign up for MyChart

To apply for MyChart access, please fill out the MyChart Access Request Form.

CHEO's Health Information Management (HIM) team reviews all applications. If we approve your application, we'll send you an email with your activation code and instructions on how to login. Please read through the MyChart terms of service.

What will I be able to see and do in MyChart?

Results for many tests are now available via MyChart soon after they have been processed and completed by the lab.

Lab results will be released six hours after processing, and diagnostic imaging and pathology results will be released at noon the next business day after processing.

At your next scheduled appointment your care team will review your test results, along with any questions you may have about them. They may add notes and comments in MyChart sooner. If the results of your test require a change in your treatment or indicate a change in your health condition, a member of your care team will contact you directly to discuss.

If seeing your results right away causes you some concern and you prefer to wait and hear them from your care team directly, please turn off test result notifications in MyChart.

What test results are and are not released to MyChart?

Released to MyChart:

• Lab and microbiology results

• Pathology reports

• Medical imaging reports

Not released to MyChart:

• Genetic results

• Sensitive results (HIV, pregnancy tests, cervical swabs for chlamydia and gonorrhea, vaginal swabs for trichomonas, syphilis, hepatitis B and C, PCR)

• Specialty procedure results (for audiology, cardiology, Gastrointestinal testing, etc.)

Some sensitive test results are only given in person or over the phone. These test results are not typically available in MyChart. If you feel your test results should be available, but you don’t see them, please contact your healthcare team.

You can view:

  • a list of health issues, allergies and medications
  • most lab and imaging test results
  • growth chart information
  • eyeglass/contact prescriptions
  • password protected PDF versions of other health information you have requested from Health Records/HIM Department
  • select letters and notes from health-care providers
  • upcoming and past appointments, along with summaries from your visits
  • summaries of visits to the Emergency Department and inpatient units
  • surgery reminders, upcoming surgery date information and surgery visit summaries

  • Send two-way, secure communications between yourself (the patient/family) and your CHEO care team.
  • Upload a photograph of yourself/your child or youth to the health record, for use by CHEO as an extra method of identification.
  • Complete any questionnaires you've been asked to fill out before an upcoming visit with your CHEO care team.
  • Send a copy of part or all of your health record to a provider outside of CHEO using a one-time share code.
  • eCheck-in for appointments, and update your demographics, primary care provider and health card information before arriving at your appointment.

Finding your way around MyChart

You can add upcoming appointments from MyChart into your personal calendar by clicking on the selecting the “Add to Calendar” link when viewing an upcoming appointment.

If you're looking to deactivate your MyChart account, follow these steps:

  • From the MyChart menu, click on Security Settings.
  • From here, follow the link in the Deactivate Your Account box to visit the account deactivation page. Make sure you read all of the instructions on this page before deactivating your account.
