Information for youth, caregivers and educators

Some CHEO clinics, services and programs allow youth to self-refer, or their caregivers and educators to make a referral on their behalf.

If self-referral is available for that clinic, service or program it will be mentioned on their page, along with instructions for how to submit your self-referral. Community providers

We are happy to share that CHEO is now accepting e-referrals to many of our clinics, services and programs!

Next time you’re sending a referral to CHEO, consider sending it via Ocean:
• It keeps you informed of referral and appointment status
• Referral forms are linked to referral criteria to make it easier to see what information is being requested
• It’s integrated with many EMRs, saving time when sharing information

Learn more about each way to submit a referral:

EpicCare Link is a secure web-based portal that allows community providers to electronically refer to CHEO and SickKids as well as have direct access to your patient’s CHEO chart.

Referrals submitted via EpicCare Link will be automatically routed to CHEO or Sick Kids.

Children, youth, families and health-care providers can now connect, request information and book referrals for CHEO mental health services through 1Call1Click.ca.

1Call1Click.ca is a bilingual service designed to match children, youth and families living in Eastern Ontario with the right mental health and addiction services, at the right time. An initiative of the Kids Come First Health Team, 1Call1Click.ca is the first of its kind in Ontario and brings together 33 local providers of child and youth mental health and addiction services.

If you are looking to make a referral to a clinic, service or program that isn’t yet live with e-referrals through Ocean, Epic CareLink or 1Call1Click, you can continue to make referrals through Fax.

Please visit the clinic, service or program webpage you wish to make a referral to, to find latest information including their referral form (if applicable) and fax number.
