Home and Community Care

CHEO was recently advised that Ontario Medical Supply (OMS), one of its third-party vendors, experienced a cybersecurity incident in April 2025. CHEO has been advised that orders placed with OMS between September 2021 and March 2025 for medical supplies used in delivering at home care were impacted by this incident. The personal health information contained in those supply orders included: patient names, contact information (home address and phone number), and details of medical supplies or equipment ordered.

Protecting the privacy and security of patients’ health information is a top priority. Throughout this process, CHEO and OMS remained focused on ensuring patients continued to receive the medical supplies and equipment they need while mitigating any further privacy and security risks. CHEO has reported this incident to the Information and Privacy Commissioner of Ontario (IPC), and an investigation is ongoing. While affected parties may file a complaint with the IPC, it is not required given the active investigation.

Many families of children and youth, including those with medically complex needs, asked for a more connected system of care. CHEO’s goal is to provide a more seamless and coordinated approach, so that it is easier for children and youth to get the right care, when and where they need it.

Pediatric home and community care services at CHEO

CHEO and partners of the Kids Come First Health Team work together to provide the following pediatric home and community care services:

  • Care Coordination
  • Nursing: rapid response nursing, shift nursing at home and in school, and visit nursing
  • Personal support services
  • Physiotherapy
  • Occupational therapy
  • Nutritional services
  • Medical supplies and equipment (where eligible)
  • Information and/or referral to other health and social services in the community.

CHEO also manages the Family Managed Home Care (FMHC) Program, which provides an option for eligible families to receive funding directly to purchase home care services or employ care providers. Find more information about the Family Managed Home Care (FMHC) Program.

Referrals for pediatric home and community care services

Health-care providers

Fill out our referral form

  • Physician or Nurse Practitioner referrals are required for nursing services (e.g., wound care, IV antibiotics).

Patients, families, schools and others

  • Patients and families can self-refer, so long as a Physician/Nurse Practitioner order is not required. Please call CHEO’s Department of Home and Community Care Program at 613-737-7600 x1794 and you will be connected to a member our team.
  • When you call, please let us know that you are making a referral for home and community care services.

If your child or youth’s family physician, community pediatrician or other health-care provider has already made a referral, one of CHEO’s Care Coordinators from the Department of Home and Community Care call you when the referral has been received and reviewed for service eligibility.

The Care Coordinator will work with you to understand what service(s) your child needs and develop a care plan.

Medical supplies and equipment

The availability of medical equipment and supplies is subject to change and we work closely with our partners to ensure timely, appropriate substitutions whenever possible. Please let your Care Coordinator know if you are experiencing any issues or have questions about your medical equipment or supplies. You can also contact us between the hours of 8 a.m. and 8 p.m. daily, 7 days a week, at the contact information below.

New Inpatient Medicine Discharge Program at CHEO

The new Inpatient Medicine Discharge Program at CHEO provides extra support for families as their child transitions from hospital to home. CHEO’s community-based rapid response nurses are dedicated to ensuring a safe, smooth discharge experience by offering guidance and answering any questions families may have. 

How we help:  

  • We provide you with guidance, education and ongoing support. 

  • We help you understand your child’s diagnosis, medications, medical equipment and discharge instructions. 

  • We help you avoid emergency department visits and preventable hospital admissions. 

What to expect: A nurse will call within one to two business days after your child’s discharge and continue to provide support until the program’s services are no longer needed. 

Program hours: Monday to Friday, 8 a.m. to 4 p.m. 
Please note, our team does not operate on weekends.
