Our Health Records team is there to help you access your health information when and where you need it. They also help keep your personal health information secure and control who can access it.
CHEO, The Ottawa Hospital and SickKids in Toronto are part of Epic's Care Everywhere network, which allows your patient health record to be securely accessed by our partner hospitals if you are ever at one of these hospitals for care. This could also include US hospitals, which means your personal health information would be leaving Canada if you seek care at a US hospital. Your health information will only ever be accessed by the health care professionals caring for you.
If you would like more information about Care Everywhere or would like to opt-out of having your information shared via Care Everywhere, contact the CHEO Privacy Office at privacy@cheo.on.ca.

You have the right to request and access a copy of your own personal health information any time. One way to view your CHEO health record is to sign up for MyChart, a secure online portal. Learn more and sign up here.C
You can now view, store and instantly share your medical imaging records through our partnership with PocketHealth. Learn more about PocketHealth.
For information that is not available in MyChart, you can request it by filling out a Request/Consent for Release/Disclosure of Patient Health Information form.
If you are requesting records for a patient who is deceased, please completed the Request for Access of Deceased Patients Records form only.
Once the form is filled out, send it to our Release of Information Team in Health Records:

Is there a cost to make a request for the release/disclosure of patient health information?
Please note that all requests are subject to fees. The fees can be found on the back of the Release form.
We encourage patients and families to pay via Credit Card by contacting the Release of Information Office at 613-737-7600 Ext. 2292. If you are paying for your information via Cash or Debit, please note that this can be done on-site at CHEO on Mondays and Thursdays between the hours of 9am-12pm. In order to reduce the amount of traffic in the Health Records Department, information can be picked up during the above times as well.

How do I request the records of a deceased patient?
If you are requesting records for a patient who is deceased, please completed the Request for Access of Deceased Patients Records form only and submit to releases@cheo.on.ca along with any supporting documentation requested on the form.
Fees will be waived for legal guardians making this type of request.
